- The appeal letter. Certified mail, claim number on top, written to your denial reason: for "pre-existing," the request for the technician's report and the plan's own unknown-condition language; for "maintenance," the records request and the specific-maintenance question; for "cap," the accounting request; and so on.
- The regulator complaint. Your state's office named, the narrative in the order regulators want it, and the attachment list.
- The timeline. One page, dated: failure, service call, technician visit, denial, appeal, response. Regulators and small-claims judges read this first.
- Arbitration or small claims. Your state's cap, whether the gap fits, what a consumer arbitration costs to start, and which one the contract lets you choose.
- What the company must do. One page from your state's service contract law where we have it, and the general rules where we don't.
